For Ichigo? That's an easy one.

At the tender young age of nine, Ichigo's mother was brutally murdered before his very eyes as the two, alone, were on a calm, peaceful walk through a park just a small walk away from their home. To this point in his life, Ichigo had sworn to protect his family, an innocent promise made as a child, so to see the very center of his universe, the person who had cared for him, whose mere smile acted as the sunshine to brush away his tears, was absolutely devastating. He wasn't even aware how she died; Masaki Kurosaki had been murdered by a terrible, soul-devouring demon (a Hollow), one that could not be seen by normal, human eyes.

To make matters worse, years later, Ichigo learned the truth behind the loss of his mother when confronted by the offending Hollow, but he was unable to avenge her when his opponent managed to escape defeat.

To Ichigo, one who prides himself on being a protector, his guilt is in his inability to save his mother. Preying on his insecurities would be simple; one would only have to accuse him of being too weak to make a difference.
