Actual Guilt: Lenny has murdered several people, believing they were his wife's killer. He doesn't remember any of the murders, so he feels no guilt for these.

Mental Guilt: He feels he's never doing enough to catch his wife's killer, and that he uses his condition as a crutch far too much. He'll never get over the death of his wife. :(
